Peer Mentoring Program

Talk with people who have experienced an injury or have a condition like yours. The Peer Mentor program helps you prepare to return to the community after your hospital stay and connects you with supportive resources.

The purpose of our hospital-based, community-led program is to meet the needs of patients with newly acquired spinal cord injuries, stroke, traumatic brain injuries, and amputee. The Peer Mentoring Program helps reintegrate you back into the community after leaving Schwab – connecting you with necessary resources to adjust to a new lifestyle.  This program pairs patients with a peer mentor from the community, someone with a similar injury and background who has successfully completed the rehabilitation process.

Peer mentors provide mentees with both instructional and emotional support, such as information on transportation, general disability resources, navigating their home, interacting with family, and doctor’s instructions to prevent secondary conditions.  Peer mentors also share their own disability acceptance, and the feelings that they themselves may have experienced after acquiring their disabilities.

The program offers both short-term and long-term mentoring.

For more information, please contact Patrick Garcia, peer mentor coordinator at 773-565-3059, patrick.garcia@sinai.org.

In My Shoes

Former Schwab patients who have experienced injuries related to street violence bring their personal stories to youth in the community. Hearing firsthand experiences from victims and perpetrators helps to raise awareness and prevent violence in Chicago communities and beyond.
